Closing Credit Cards Without The Credit Card Number

Here's a question I received via email:

After I ordered my credit report, I saw numerous credit cards with low limits that I haven't used in years. I would like to close some of these credit cards, but I no longer have the credit card number. Only a few digits of the credit card number are printed on my credit report. What's the best way to get the credit card numbers and close these accounts?
